The Aboriginal Supportive and Community Care Coordinator role will;
The Aboriginal Supportive and Community Care Coordinator role will;
- Support Aboriginal clients and their families using SESLHD services with Advanced Care planning and access to palliative and end of life service.
- Engage local Aboriginal communities in the development, delivery and evaluation of culturally appropriate initiatives and strategies that provide positive Palliative and end of life care outcomes.
- Collaborate with SESLHD Palliative Care services, primary care and other health and social care services to develop culturally appropriate, integrated and comprehensive care plans.
- Build mainstream services’ understanding of local Aboriginal communities and their needs around Cultural practices.
- Be of Aboriginal or Torres Strait Islander descent (An applicant's race is a genuine occupational qualification and is authorised under section 14(d) of the Anti-Discrimination Act 1977).
- Holds a minimum Certificate III qualification in Aboriginal Primary Health Care or relevant qualifications/experience in an appropriate health related field.
- Demonstrated understanding of issues affecting Aboriginal people in relation to grief and loss and Aboriginal social and emotional well being.
- Experience in community engagement and/or development with Aboriginal people .
- Knowledge of advance care planning and end of life services.
- Demonstrated ability to work independently and as part of multidisciplinary team to develop culturally safe environment for Aboriginal clients and their families.
- Demonstrated excellent written and verbal communication skills with the ability to develop culturally appropriate resources and document in the client’s electronic medical record.
- Current unrestricted driver’s licence (with a willingness to travel in accordance with the demands of the position).
